Since 2008 we have helped Medicare beneficiaries across San Diego County understand their options, enroll on time, and choose a plan that keeps their own doctors and pharmacies in network. You are never charged for our help — we are compensated by the insurance carriers, not by you.

When you can enroll in Medicare

Missing an enrollment window is the single most expensive mistake we see. These are the periods that matter:

  • Initial Enrollment Period (IEP) — the seven months around your 65th birthday: the three months before, your birthday month, and the three months after.
  • Annual Enrollment Period (AEP) — October 15 to December 7 every year. This is when you can switch Medicare Advantage or Part D plans for the following January 1.
  • Medicare Advantage Open Enrollment — January 1 to March 31, for people already in a Medicare Advantage plan who want to change it or return to Original Medicare.
  • General Enrollment Period — January 1 to March 31, for people who missed their Initial Enrollment Period.
  • Special Enrollment Periods (SEPs) — triggered by events such as losing employer coverage, moving out of your plan’s service area, or qualifying for Medi-Cal or Extra Help.

The parts of Medicare, in plain language

  • Part A covers inpatient hospital care, skilled nursing and hospice. Most people pay no premium for it.
  • Part B covers doctor visits, outpatient care, labs and durable medical equipment, and carries a monthly premium.
  • Part C (Medicare Advantage) bundles Parts A and B, usually with drug coverage and extras, through a private insurer.
  • Part D is prescription drug coverage, sold either on its own or built into a Medicare Advantage plan.
  • Medicare Supplement (Medigap) plans sit alongside Original Medicare and pay the deductibles and coinsurance it leaves behind.

What Medicare costs in 2026

For 2026, the Centers for Medicare & Medicaid Services set the standard Part B premium at $202.90 per month, with an annual Part B deductible of $283. The Part A inpatient hospital deductible is $1,736 per benefit period. Higher-income beneficiaries pay an income-related adjustment (IRMAA) on top of the standard Part B premium, starting above $109,000 in income for individual filers and $218,000 for joint filers. If your income has dropped since the tax year Social Security used, that adjustment can be appealed — we can show you how.
